2009 Camp Health Form for Youth & Adults  This form will need to be filled out and handed back to your den leader no early than April 15,2009 if you are planning on going to Day Camp or Overnight camp.  Please be sure to sign this form as no one will be permitted to stay at camp without this signed health history form.

Campership Form  - No Cub Scout should be kep from a summer camp experience because of lack of money.  Use this confidential form to apply for a campership.  The camper portion of at least one-half of the camp fee must be turned in with the campership application.
